I am able to reproduce the AFS missing file problems (with subsequent
likely kernel oops) with the following procedure on a 9.4.11 machine:

1) log in remotely via ssh without tickets
2) run a shell script (or any executable?  or just access a file?)
3) let tickets expire
4) try and run something (even unrelated, I think; perhaps the directory
   traversal is what's important)
5) renew tickets
6) try and access the aforementioned file again

If more information or access to a losing-but-not-oopsed machine would be
useful to anyone (Garry?), let me know.  I've seen this a few times now
and the common factors (some of which seem irrelevant, but who knows) are
as above.
