I've seen this on geskekelud (a Sun) also. I haven't been able to come up
with a confirmed way to replicate it, but it seems to be somewhat linked
to logging in on one machine, logging in on a second machine, messing
around with the panel, logging out of the second machine, and then logging
out of the first machine.
