If RVD is not enabled in a kernel built for the VAXSTAR, the major
device numbers for the block special devices for the disk will be
incorrect. This is because of an error in the #ifdef's for the RVD
device entry.
[It doesn't leave another null entry in its place].
Look at /u1/sys/vax/conf.c for an example of how to do it right
[warning: the code there also has some unrelated differences.]
