The bizarre footnote behavior you describe does not surprise me.
It turns out that footnotes push the troff back end print processor
to its limits and sometimes tickle bugs that require AT&T to admit
they have a product about and fix.  It is impossible to get troff
bug fixes from AT&T for troff any more.

We were going to fix this class of bugs by migrating to using GNU troff,
but we never got the warm bodies to do the work.

CMU has rewritten EZ in C++ to bypass troff and format directly to PostScript
but I have not tried out this new version yet.

There _IS_ someone who cares about EZ bugs, but only as much as, say, the
Frame maker people care about their bugs.  :-(

I see that this bug has escaped my attention for far too long.  I apologize.
Both you and the user deserved a reply weeks ago.  I'll try to do better.
