The form is located at the following address:

When you click on either faceplate (the number 7506 or 7507 is the link) on
that form it will bring you to a different page (and anchor). Depending on
which faceplate you click on you will reach:
http://web.mit.edu/is/tel/csrform_break.html#7506
or
http://web.mit.edu/is/tel/csrform_break.html#7507
7506 and 7507 are the model numbers of 10 and 20 button digital telephone
sets. Most people will be more concerned with a 7506 faceplate. Please
let me know if this remains confusing and I will try to re-work it.
