Hi Janet,
I just finished adding two fields to the breakdown page of the form that
may help answer some of your questions.  Per Robyn's suggestion I added an
example of each telephone style's faceplate and filled some of the fields
in.  When you click on either faceplate on the form it brings you to the
examples.
I just got the authorization to post this form yesterday so I will be
looking for feedback from the supervisor of the customer service department
for various items on this page.  I need her to make some decisions on the
content and processes that lie behind submitting and receiving these forms,
so we won't be able to post it for a while.  Please let me know of any
other concerns you may have regarding this form (or any of the pages I've
done so far).
