This is what we use. We are a "light repair" shop.  We do not open computer
cases for any reason and we do not format harddrives/install OS's.

 

 

Clarion University ResNet Support Center

Computer Repair/Support Request and Release Form

 

Service Authorization  and Terms of Use

 

I, _________________________, give the Clarion University ("CU") ResNet
support center permission to work on my personal computer in my absence.
This CU Repair/Cleaning service is a "Best-Effort" service provided by the
CU ResNet Support Center to its customers at no cost.  I understand that the
CU ResNet Support Center cannot guarantee a solution to the problems the
personal computer is exhibiting, and I hold CU Resnet Support Center, staff
members, Clarion University, and its employees harmless in the event that
further damage occurs as a result of the attempt to remedy existing issues
with my personal computer.  The CU Resnet Support Center cannot guarantee a
date or time when the personal computer will be available for pickup, and
will contact me when the personal computer is ready.

 

The CU Resnet Support Center staff will store the PC in the CU ResNet
Support Center Office until the service is completed.   The CU ResNet
Support Center makes no guarantees or estimates as to when the equipment
will be ready for pickup.  If the CU ResNet Support Center cannot resolve
your specific problem, we may refer you to a list of vendors who may be able
to assist for a fee.   There is no charge for the services provided by the
CU ResNet Support Center.  This service is provided to assist students with
their technology needs.

 

Since your equipment is not University owned, all equipment must be picked
up by the owner within 14 days of being notified by phone or email of
completion. The CU ResNet Support Center will contact the owner of the
system with a warning 24 hours before disposal.  When picking up any
equipment the owner must show their valid Clarion ID. Any equipment not
picked up within the 14 day period will be considered abandoned.

 

NO WARRANTIES: TO THE EXTENT PERMITTED BY APPLICABLE LAW, NEITHER CLARION
UNIVERSITY ("CU"), NOR ANY PERSON, EITHER EXPRESSLY OR IMPLICITLY, WARRANTS
ANY ASPECT OF THIS SERVICE, INCLUDING ANY OUTPUT OR RESULTS OF THIS SERVICE.
THIS SERVICE IS BEING PROVIDED "AS IS", WITHOUT ANY WARRANTY OF ANY TYPE OR

Subject: Language Request

 

Penn State is in the process of reviewing the indemnification language we
use on forms that we have students sign before we touch their computers.
Currently the language covers the issue of data loss but does not discuss
any other area, such as hardware failure.  Given the critical view students
have of their computers and the litigious society in which we live, we are
striving to make sure we provide the level of service that is expected but
protect the University from unfounded legal action.  I would appreciate any
sharing of language that other schools use on the same types of forms they
have students sign for normal trouble shooting as well as more complex
actions such as formats and rebuilds.

is in the process of reviewing the indemnification language we use on =
forms that we have students sign before we touch their computers.&nbsp; =
Currently the language covers the issue of data loss but does not =
discuss any other area, such as hardware failure.&nbsp; Given the =
critical view students have of their computers and the litigious society =
in which we live, we are striving to make sure we provide the level of =
service that is expected but protect the University from unfounded legal =
action.&nbsp; I would appreciate any sharing of language that other =
schools use on the same types of forms they have students sign for =
normal trouble shooting as well as more complex actions such as formats =
