Hello,
exmh has an odd bug which prevents the "ispell on" box from being
enabled _until_ you've composed at least one message. Try hitting
"comp", then aborting the message, and then try to turn on ispell.
(This is on my list of things that exmh doesn't quite do correctly.
The list is, unfortunately, dusty.)
If ispell still can't be turned on after you've comp'd a message,
let us know.
