I might be able to help with the host name thing...
When you upgraded you might have clobbered /etc/hosts
This file is used to lookup names and turn them into IP addresses:
111.111.111.111 aptiva aptiva.fred.com
...Would be an example entry,
By default,
127.0.0.1 localhost
...is almost always there.
To fix it, you'll need a backup of you hosts file, OR a domain name
server and a properly configured networking setup, OR the IP number for
aptiva so you can manually enter it into /etc/hosts.
Your username/password utilities appear to be munched.
You did an UPGRADE ?
I've not migrated to 4.0 so hopefully someone else can elaborate from
first-hand experience...
