The convertor might be your problem. Please give exact details of
SCSI bus topology, host adapter configuration (ie termination ON/OFF/AUTO/
HIGH/LOW), especially cable width, etc.

did you obfuscate this, or was this reported as is ?

This doesn't look like a wide disk/host, non wide cable problem
since it didn't crash in a data phase. However it could be
bad, inexistant or double termination.
