SCSI medium error

Hello,

 When I run the command "dd if=/dev/zero of=/swap bs=1024 count=8192"
the machine effectively locks up, and I have to physically reset it. 
When I reboot and run e2fsck  I get the following error:

e2fsck 1.10 .....

SCSI 0 Medium Error on channel 0, id 0, lun 0, CDB Request Sense 00 00
00 10 00 Current Error sd08:02: sense key medium error Additional
sense indicates unrecovered Read error scandisk I/O error  dev 08:02
sector 689984

Pass 3: ....
Pass 4:....
etc..

Could someone tell me what this means?  Is my hard drive damaged? my
machine locks up after this happens.

I am running RedHat 4.2 on an i586 100mhz, isa adaptec SCSI 142x
controller, quantum Fireball 1.2GB SCSI-2 HD
