2.0.3x kernels seem to be very buggy regarding virtual memory handling.
I suggest running 2.0.27.
Also note that the Quantum Fireball have a bug pertaining to command
queuing, thus if your driver is using it, disable it.

This is a hard error with a SCSI device.
