I have reworked a number of papers in exactly the way you have described,
essentially the problems can be obviated by the use of a good HTML
publishing wordprocessor that can handle a twain compatible device.
Where problems arise are with manually having to check the accuracy of
the OCR and then the actual formatting in a presentable way, of the images.
I estimate that you will need an above average scanner to do it quickly . I
used a genius flatbed 2400dpi and it took about 30 minutes a page from
start to finish scanning OCRing and image embedding. Not for the faint
hearted...
