If anyone's still following this, the PDF I mentioned is at:

Interestingly, I can no longer reproduce the problem. I was trying to
print to a CUPS print server that is no longer set up; I assume
something about its configuration must have been relevant somehow too.
(Though the server had no issues with any other documents, nor with this
document when printed from acroread.)
