Hi Rocklyn,

Will the BLT and/or CHD have the ability to see what data is entered on 
behalf of a person receiving this type of e-mail?  If all three pieces of 
information is required, does the form check that all information is 
entered before accepting any data?  Also, will there be a timestamp 
associated with the data?  I remain concerned about the relationship 
between the data and its actual use.

Example:  I am planning to relocate from W92 in the near future.  If I 
complete the form today and several months later request a report via this 
service w/o updating the form, my request may not reach me.  I may not 
remember to update the data.

I am not sure that the use of "valid" cost object and "valid" building and 
room is accurate since no additional verification is being done of this 
data.  Or, did I misunderstand?
