protocol error with registration applet

Hi,
The user John P Costanza has been trying to register the username jackc
for 2 days. He's able to register the username, however when he enters
the password and clicks continue, he gets the following error:
"A protocol error has occurred, and the registration applet is unable to
continue. This may indicate that you are using an out-of-date version of
the registration applet."
Then it instructs him to contact us.
He tried this with netscape communicator 4.05 in N43 and here. I was able
to register someone else on this machine earlier today so it appears it
may be related to his moira entry.
I "guessed" it might have been that the principal was already in use
(though I remember it giving a different error message) but jackc seems to
be available.
Any ideas?
