I have made the changes discussed at kaKApo. Please take the time to
review the forms and result pages. You can submit the form if you'd
like. Mail will go to you (i.e. for now the form sends e-mail to the
address/username you specify, for testing -- in production it will send
mail to hesreq or kakapo).
Note that the mail queue on web.mit.edu still seems to be majorly
backed up. If you don't get mail after submitting the form, that's most
likely why. Look for it sometime over the next three days.
