When an RVD server gets hard errors on a raw device from which it is
exporting RVD packs, these errors can cause workstations using those
RVDs to crash, even if they don't have the pack mounted as a file
system. (I was using 'dd' from an unmounted pack, and my workstation
died.)
My microvax crashed in vdbad() twice, but didn't provide a crash dump
either time.
