One possible solution to the off-net-18 "problem" would be an IP-over-IP tunnel along the lines of what the "mobile IP" people are discussing. Implementing such tunnels would also allow you to provide "link-level" encyrption to secure your traffic from the prying eyes of other CATV subscribers...
