Cannot find something in the IS web pages

** Where did you expect to find it? I thought if I went to
http://web.mit.edu/is/services/list.html#telephones I'd see something about a form to apply for service. I can't see it.
I had the URL for the explanation of the form page (http://www-mit.mit.edu/afs/athena.mit.edu/org/i/is/tel/explanation.html) but couldn't find a link to the form. I finally was able to get into http://www-mit.mit.edu/afs/athena.mit.edu/org/i/is/tel/ and locate a form file, but the form I found seems to be the student form? (acus)???
Another question, the explanation says the form must be sign by a person in charge. How does one sign an online form?
Suppose I was a real user, would I have an easier time?
