Hi,
The pdf format will work on the 3 platforms. With PDF format, the user
would either need to print out the form and complete it by hand or convert
is to another format (such as word or wordperfect) in order to edit it
online.
To save people from having to do their own file conversion, the form could
be saved in a few different formats (such as Wordx.x, etc...) Different
links can be made for each file format (eg: "If you have MS Word version
x.x or higher, click here to download form"). Since a lot of people like
to complete these forms electronically, they probably should put up a
few major formats plus pdf for those who use less popular word
processors. ;-)
