1999 PA form for Competency Group

Hello Everyone,
I just had a meeting with Anna Socrates re: posting the 1999 Performance
Appraisal form on the web. The Competency Group would like to make the
form available for downloading to Macs, Pcs, and Unix stations as a Word
document (or something similiar). The user should be able to save the form
to their machine and be able to fill out and print it at review time. I
was wondering if anyone had any suggestions as to how this would be done
(PDF file?). She has also contacted cwis-help for their advice. I believe
that the CG has given her a deadline to have this done so any input or
suggestions before Friday's meeting would be greatly appreciated, otherwise
could we make it a brief topic of discussion for this week's meeting?
