bad disk on fileserver at pika

I'd like to report a bad disk on pika's fileserver adam.mit.edu
(serial number WF62110488 i.p. address 18.141.0.3) The ra1c disk is
going bad very quickly with over a dozen hard errors arising over the
last three to four weeks or so. I have made attempts to fix these hard
errors using rqbads but the errors keep occuring and do so in the same
areas with the block numbers being very close.
I am writing to request that this disk be changed before the present
disk becomes very bad. I'd be happy to provide you with whatever
information that is necessary. My telephone number at pika is 492-6983
