problems with decstation3100 from athena

The DECSTATION 3100 delivered here for james anderson now is *really*
not working. When it first arrived its disk 0 had what looked to james
like low level read errors (this was the /usr filesystem, I believe).
For a while today it booted far enough to be able to run /etc/version,
which announced that this was an "Athena Workstation (MIPS) version
6.2A May 26, 1990."
Now, it is worse:
0) at the monitor prompt I test the devices with test -c and everything
passes.
1) I type either 'auto' or 'boot', get all the stuff that would be in
/etc/dmesg, including:
WARNING: no remote file system (missing /RFS/.LOCALROOT)
and then this happens:
panic: ialloc: dup alloc
inum=5 fs=/ mode=0100666
and a lot of register dumps. Then I get returned to the monitor.
I can't make it not do this, so I can't analyze it any further.
I think:
0) disk 0 and perhaps disk 1 need to be checked for hardware problems
1) /etc/fstab needs the right line to mount disk0 (we now have no
idea what partition is supposed to be mounted or on what filesystem.
2) perhaps it needs to be reloaded with the current version of athena
software (regardless of how busy the system people are) and sent
back here.
