Communications Week ran an article in the 4/25 issue about ISDN data fees.
Essentially, almost all the major local exchange carriers apply a usage
fee for ISDN service.  According to the article, of the two carriers which
don't currently have usage fees, Southwestern Bell is considering adding
them, although BellSouth is apparently not.

The article goes on to argue that usage-sensitive pricing structures serve
as barriers to entry-level usage, and affect the popularity of ISDN.

They offered the following table:
