I did this about 3 years ago... Had a bunch of radio buttons and a
couple of free form comment boxes... It was done in PHP and linked to a
Mysql database which stored our resnet info... Each student could only
take the survey once, and then it set a flag on their record that they
had taken it... I then set up a "results" page so other people could see
what their peers had written. Most of the comments where pretty good,
but you will always have the people that do it as a joke. So I had to go
and "censor" some of the free form comments.
we had about 70% of the current resnet users answer it I believe...
