I haven't seen modern WiFi cards that don't work properly, we have TTLS
(read: complex) security here and I drag in all sorts of hacked, cheap
hardware. I would try to bump back to 'standard' 20MHz bandwidth and set
the basic rate to 'auto' if possible. I don't like having B/G/N running all
at once; if you can, I would go G-only on the 2.4GHz band, N-only on 5GHz.
B-only stuff hasn't shipped in a while.

WiFi is like having a hub, only worse. In some cases I'd rather have two
50mbit hubs loaded with clients than one 100mbit one, more collision
domains can be better, even if it sometimes means lowering peak bandwidth.
If your APs have dual antennas and support diversity, you can sacrifice
maximum individual client bandwidth and instead have two totally different
channels going simultaneously, which might be better in a busy environment.
Also, I imagine it's really hard to find 40MHz free in some circumstances,
your APs might be stepping all over each other.
