We used to have a set of addresses for which P2P traffic was blocked by
our Packetlogic. We would assign the computers cited in DMCA notices new
addresses within this block, and send the individual a letter of warning
and require a reply. But the P2P block would remain in place until the
end of the academic year. Repeat offenses would be referred to judicial
procedures. Repeaters almost always had to be caught using a different
computer than the one originally blocked ;-)

Now we've inverted the logic, blocking P2P for all and providing a means
of requesting an exemption (no questions asked). The number of
exemptions requested has been pretty small. One of our exempted
individuals was the subject of a recent DMCA notice, and we're removed
the exemption and added their userid to a "blacklist" for whom
exemptions cannot be granted.
