Hello Larry,
We're in midst of writing our own survey builder that we intend to use for =
our quality of life survey. It's actually under-going beta-testing right n=
ow with no problems, even though we're still working on implementing Likert=
scales.
Some of the restrictions with Survey Monkey were a downside, and we really =
wanted something that we could tweak on our own. It's open-source and host=
ed on our github page: https://github.com/mostateresnet.
If you have the resources and a little bit of experience managing apps, it =
should be more flexible than survey monkey. I had hoped to present on this=
topic at the Student Technology Conference<http://resnetsymposium.org/>, b=
ut I'm afraid I won't be attending this year.
As far as the questions go, we've typically had a section on the quality of=
life survey that asks general questions about whether the students' techno=
logy needs are being met, their satisfaction with the speed of the Internet=
, if their printing allotment is adequate, and if the computer labs meet th=
eir needs. We also have a section for general comments, which has proven i=
tself to be very valuable.

We're planning to start using a satisfaction survey as part =
of our Authentication Login screen. My plan is to make it voluntary and kee=
p it very short. 2 or 3 questions and a text field for comments, questions =
and contact information. We have a license for survey monkey, so we'll use =
that unless someone has a better option. Is anyone doing this? What kind of=
questions do you ask? Are you getting much response? Using something besid=
es survey monkey? Thanks in advance for any response.
