ResNet Agreement for Services
A. OFFER
The University of North Dakota ResNet office offers to furnish limited technical support for computers owned and operated by students living in campus housing.
B. STUDENT RESPONSIBILITY
1) I understand that it is my responsibility to backup data, software, and personal correspondence that I wish to retain as a record of information from my computer. ResNet staff members will not backup data, software, or personal correspondence unless given permission to do so.
2) I understand that ResNet staff has a responsibility to report any evidence of illegal activity to the proper university authority (UND Housing, UND Police, or the Dean of Students).
3) I understand that ResNet is not liable or responsible for any damages, loss of equipment, and/or loss of data that may occur while my equipment is in its possession, including unforeseen acts of nature. Students are encouraged to carry renters insurance to cover personal belongings in case of damage or loss.
4) I understand that I must purchase any necessary components required to repair/upgrade my computer.
5) I understand that ResNet staff members may recommend parts for me to purchase and that I will not be reimbursed if the recommended part(s) does not fix the problem. Additional recommendations may be made to resolve the problem, and I understand that I’m not obligated to continue service with ResNet.
6) I understand that it is my responsibility to retrieve my computer equipment and that the computer equipment will not be delivered to me under any circumstances.
7) I understand that if my computer equipment is not picked up within 10 days, I will be notified by phone. If I have not picked up my computer equipment within 2 days of the phone notification, I will receive written notification. The written notification will include a pickup deadline. I understand that, if I have not picked up my computer equipment by the date provided on the written notice, my computer equipment will be considered abandoned property.
8) Student computer equipment abandoned for more then 30 days becomes the property of the UND Department of Residence Services to dispose of according to university policy/procedure.
By signing below, I represent that I am the party that owns or is directly responsible for the property for which I am requesting repair, and that I agree with the terms of the ResNet Agreement of Services.

I understand that any disagreements with the condition of the equipment must be clearly voiced at the time my equipment is returned to me, otherwise I am waiving any right to claim damage done to my equipment while it was in the possession of ResNet.

Hello,
During the conference this past summer I talked to several of you who have drop off services. When students bring in their computer to drop it off you have them complete and sign a liability form. Would any of you be willing to share such form so I can make sure that I am not missing anything in the one that I am creating?
