recent outage?

At the meeting, Tom mentioned that he thought that there had been an
outage that hadn't been mentioned in mail to sipb-afsreq. I said
that I thought that mail had been sent, and then later said that I
wasn't so sure. I just finished looking at my back mail, and I'm
still not so sure. Can anyone clarify, please?
