Here's what I did:

+ (add an entry)

all Carriage returns (no other fields)
Add new entry to rolodex? y
Back at the TOP LEVEL MENU
% (scan the rolodex by entry)
Name: Matthew Gray
Date Updated: Tue Dec 1 16:11:09 1992
RETURN : continue the scan. % : scan rolodex from this point. + : update or add to the entry. < : display previous entry.
- : delete the entire entry. \ : abort the scan.
Action (? for help) : +
Then...

Segmentation fault
This happens 100% of the time (4 attempts).
