I think that there ought to be two sides of files.  Question files and topic
files.

Each question file ought to have 1) question 2) reference number 3) choices
and 4) responses for the question

Each topic file ought to have a list of questions that topic is associated
with.

If you ask for a question corresponding to the topic, it looks up the topic
file and randomly chooses a question in the file.  

The nice thing about this format is that you can invent new topics using
existing questions.
