I pronounce it the same way no matter where in the word it is. Unvoiced.
Fricative. With two streams of air running out the sides of my tongue,
so that my lower lip can feel it. I've never had a particular problem
putting it at the end of a word.
About 2 glottal stops, I just pronounce it twice as long. Just as when
you pronounce two, say, t's, you pronounce it twice as long.
(In English, that happens in "cattails" and "misspell".)
The "echo" only occurs at the end of a word.
