3.0.3 to 4.0 harddrive update woes

I've searched the archives and FAQs but haven't run across my problem
yet.

I booted with the bootdisk, changed to the supplemental disk when
prompted.  Got the following on the screen:

  294 blocks
  in second stage install

The install-or-upgrade screen appeared, I selected upgrade.  I
answered "yes" to scsi adaptors (I actually have 2: one narrow ncr-pci
with a cdrom and 2GB HD, and an adaptec 2940W with 2 1GB wide drives)
and selected the ncr and autoprobe.  It scanned the disk and found my
linux partitions:

  /dev/sda3
  /dev/sda4

I selected my old root partition, /dev/sda3, and got the following
error screen

  Mount Failed: no such device or address

Acting like nothing happened, the install program continued and asked
which partition the RedHat directory was in.  After selecting
/dev/sda4, I got a screen which said

  Scanning packages

and then a load of error screens saying

  package XFree86 at line 308 dne
  package xfm     at line 339 dne
  package xpm     at line 340 dne
  package xpm     at line 344 dne

(and on and on)

What am I doing wrong?  The install can't be this hard, can it?
