How to build a new bootdisk?

Hello,

I'm trying to install RH-4.0 from Mitsumi FX-100D CD-ROM. 
However, it does not work with the included mcd.o driver:

  <6>mcd=0x340,11: Mitsumi Status Type and version: 10 D 4 Double Speed CD ROM
  <4>mcd: I/O error 0x54 (Read error) Speed now 1x
  <4>mcd: I/O error 0x54 (Read error)
  .
  repeated 5 times
  .
  <4>mcd: read of block 16 failed, giving up

I already tried the modified mcd from ftp.readhat.com/johnsonm/,
but still no way. However, using DOS or the mcdx.o driver I can
use this CDROM.


So my question is, how to include mcdx.o in a self-made
boot disk? I do not like to use a hd-install.


It seems to be somewhere burried in misc/src/trees, but how to
use these scripts?
