I think I just figured it out now.  Any chance that you entered a
*simple* password that the passwd program wouldn't like into the
user tool?  If so, I think it didn't realize that the passwd program
rejected the password.

That's automatically been fixed with the latest version of pam that
we are about to release; root will now be able to set any password
without complaint, and that will fix usertool as well.
