I've been having similar problems, but I got playmidi and xplaymidi to
work when I specify the -a option (output to awe32 wave synth):
playmidi -a whatever.mid

I have this exact same problem, and for the life of me I can't figure
out why it's happening.

This works for me as well, but I agree it's not all that desirable an
option (neither is playmidi or xplaymidi in my opinion). I've also had
problems with kmid and kmidi (from KDE).
If you figure out anything more let me know.
