Errors reading from my floppy drive

I'm trying to copy files to my floppy using mcopy, but I keep getting the
following error:
fd0a: hard error reading fsbn 16 of 0-35 (st0 44<abnrml, top_head> st1
4<sec_not_fnd> st2 10<wrong_cyl> 0 head 1 sec 1.
Can anyone tell me what the problem is?
BTW, mdir doesn't work either.
