You probably have a problem...

Well, I don't know the mylex controller and the ADIC tape, but I've got an
adaptec 2940 on an IBM compatible PC with a SONY DAT tape drive. When I
boot my PC, the controller bios scans all SCSI devices and displays them.

You probably (again) want to check SCSI ids, bus termination etc...
