SCSI Problem..

I was hoping someone could point me in the right direction as to how to
solve this problem we're having... We have an Adaptec 2940-U2W card
connected to a Toshiba SCSI CD-ROM and a WD LVD 9gig drive.. under both
2.0.36 and 2.2.3 the machine locks up (to the point you can no longer
telnet to it as well as you can no longer log in, although you can
switch VCs).. the message displayed is similar to (had to write this
down by hand):

(id 0=HD, id 4=cdrom)

SCSI bus is being reset for host 0 channel 0
SCSI error: host 0 id 4 lun 0 return code: 26030000
sense class 0, sense error 0, extended sense 0
scsi0 channel 0: resetting for second half of retries
SCSI bus is being reset for host 0 channel 0

or

SCSI bus is being reset for host 0 channel 0
SCSI disk error: host 0 channel 0 id 0 lun 0 return code = 26030000
scsidisk I/O error: dev 08:01, sector XX (XX changes)
scsi0 channel 0 : resetting for second half of retries

and this happens repeadetly until it seems to lock up.  The machine
itself is a Pentium III w/ 768MB ram, on a Gigabyte GA-6BXE
motherboard..
Any pointers as to where I can look to find information on helping my
solve this problem would be most appreciated.
