You should have *verified* the disk and replaced the bad block. This
option is available in most SCSI BIOSes (Tekram, Adaptec, Qlogic).
People tend to use FORMAT UNIT to *hide* problems, not to solve them.
Worse, some implementations do not preserve the grown-badblock list.
