  The early NEC 3Xp CDROMs had a firmware bug which caused occasional bus 
  hangs and rather-more-than-occasional inability to find audio tracks.  
  Sounds like the buggy code got recycled...

A bus hang would be perfectly consistent with the observed behavior shown in
the log excerpts.  The BusLogic controller is apparently also tied up, which is
why the "OGMB still full" message occurs.  The present BusLogic driver, despite
all the messages about sending RESETs, is not actually doing anything, so it
cannot recover from this condition.
