I have had the same problems lately... Like when I try to change blocksize
using "mt" on my exabyte 8500, it sometimes locks up the scsi bus
completely. I can hear from the tape drive whirring that it is trying to
reset the bus and it keeps writing stuff to the console (if I can switch
to it) but it never gets out of it. Booting single user and just doing the
block change works ok.

It is kind of annoying as I have everything on this bus (4
widescsi-drives, one narrow scsi, a cdrom and the exabyte 8500) and when
it locks up all I can do is reset.

I have a 2940UW with bios 1.21. I have the wide scsi drives externally and
the narrow units internally.

My experiences are that as long as I dont touch the tape drive, everything
works perfectly. As soon as I start to touch it, there are chances it'll
freeze the scsi bus. Especially if it involves changing block size.
