printer repair request

Hello,
    I need to place a hardware service call for a printer in our study room 
in the Penthouse of E55 {ph on the elevator button}. The printer is an HP 
5si-mx and is located on jack # 2907-103, port #2. The problem is tray # 2 
is jammed and will not open. I used to be a computer repair technician in 
another life, so I took a look at it to make sure it wasn't something 
simple. It is not, it needs to be it repaired.
     I was instructed to submit this request by our RCC. Please let me know 
if this is not the proper way to submit this request.
