The culprit here seems to be when we run synctree to make a symlink farm
in the IRIX on-line books tree (/usr/share/Insight/library).  I tried
running this now against the dev cell RO insight volume, and its access
count increased by 100,000.  The tree contains roughly 10,000 links in
800 directories, so that count seems higher than might be expected.
