NLM has just published NATIONAL LIBRARY OF MEDICINE RECOMMENDED FORMATS
FOR BIBLIOGRAPHIC CITATION, by Karen Patrias.  It's available from NTIS
(PB91-182030) for $27.

It contains formats used in the production of Medline, and recommends
bibliographic elements to include, order, and punctuation.  It gives
various examples of each type.  Electronic formats covered include
computer programs, databases (entire or in part), serials in electronic
format, bulletin boards (entire or contributions), electronic mail.

I said just published; it came out April 1991.
