Need Help on Adaptive Technology Thesis

I am a graduate student at the University of North Carolina at
Greensboro in the MLIS program. I am also visually impaired and hearing
impaired. I'm conducting a survey of end user opinions of adaptive
technology in libraries.
I need the names and mailing addresses of all types of libraries in the
United States that have adaptive resources for use by blind and visually
impaired patrons. This could be as simple as a resource room or as high
tech. as an online talking catalog. I wish to contact the directors and
ask for assistance in conducting this mail survey and the number of
copies needed in the appropriate alternative formats.
Your assistance will be acknowledged in my paper which I hope to
publish upon completion.
