Well, we definitely don't want to use .ph headers. I've been wanting to
get rid of those for ages, and eventually perl is likely to just punt
them.
I thought there was some more POSIX-ish way of getting at the window
size than ioctl(), but it looks like I was wrong. So parsing "stty"
output may be the only option.
