That's what we call "operator error" and it is responsible
for a great deal of avoidable damage.  Not surprising,
though.

Not necessarily. Several of the dislocated joints and injured muscles were
incured when a race car I was driving blew a tire on the back straight away
at about 120MPH. The tires were brand new.

Shit happens, no matter what you do.
